Misty A. Spurgeon, 26, the driver of one of the vehicles, was pronounced dead at the scene, troopers said.
The crash occurred about 11:20 a.m. east of Dutch Road in Waterville Township.
Ms. Spurgeon was westbound when her car was struck head-on by a pickup driven by Adam D. Straub, 25, of Columbia City, Ind.
Mr. Straub started to pass a tractor-trailer rig, lost control, and hit the driver’s side of the tractor. The pickup truck went into the path of the Spurgeon car.
Mr. Straub and his passengers Mason Straub, 1, Madison Straub, 4, and Andy Straub, 22, and the rig driver Nicola Utjesinovic of Hamliton, Ont., were not injured.
